The Center Toward Self-Reliance is hiring a San Luis Valley Case Manager!
The goal of our agency is to help people with disabilities to live independently in the community of their choice. We accomplish this task by working with the consumer to craft an independent living plan that will help them to become as self-reliant as possible. This position facilitates that focus by acting as a "gateway" to general services and a channel to some of the agency specialty services as needed.
Some of the specialty areas include youth transitions, nursing home transitions, low-vision services, deaf services and employment services. The position requires that the hired applicant complete weekly outreach (about 8-12 hours per week) with other agencies and businesses throughout the San Luis Valley, so travel is involved in that region.
Generalists must have a dependable car and auto insurance. Mileage will be reimbursed at the state rate. The position pays between $19.50 to $21.00 an hour depending on experience and education. A generous time off policy and a stipend in lieu of insurance is offered as well.
The right candidate will be

- work 20-28 hours per week to begin with the potential for full-time work, once an appropriate caseload is established.
